JAA Intent to Enroll

Hello, and welcome. Congratulations on reaching the final phase of completing the CCRI portion of your JAA program.

Please read all these instructions and complete this entire form.

Please pay special attention to the link at the bottom before you sign the document.

Important: Upon completion of this form, you must immediately request an official electronic transcript to be sent to transferadmission@uri.edu. We will not process your intent to enroll until your electronic transcript is received. To request your transcript please visit: https://www.ccri.edu/oes/records/onlinetranscript.html

After you submit this form, the URI Admission Office will process your Intent to Enroll. Your official URI Admission acceptance letter will include information about paying your enrollment deposit of $300. This deposit will be applied to your first-semester tuition. Information about on-campus housing will also be included in your acceptance letter.

You will also receive an email from URI's Transfer Resource Center. This email will include website links and important information about advising and course registration. It is important that you follow all instructions and pay your enrollment deposit ASAP so you can register with URI juniors. Thank you, and we look forward to you joining our URI community!

I understand that I can change my JAA transition plan while a student at CCRI but the graduation requirements will change and alter my graduation/completion date.
I understand that I may be eligible to receive a JAA Academic Achievement award based on my Cumulative CCRI GPA. Exact GPAs and tuition discount percentages are subject to change, but you would receive ample notice. I further understand that I MUST graduate and send an official transcript to URI no later than 12/31 if you plan to start at URI for a spring semester. January, or J-Term graduates are not eligible for a discount.
I understand that I should prepare to transfer but not graduate with the AA, earn specific college GPAs, or complete mandated courses that the JAA Achievement Award will be void without an appeal. I recognize other scholarships and pathways into URI may be available to me.
Once at URI, I understand that I must maintain at least a 3.0 cumulative GPA to retrain any/all awards. Your URI cumulative GPAs will be reviewed in December and May for the following semester. Summer or J-Term classes influence your cumulative GPA but will not be considered after the May and December reviews.
I understand that my JAA achievement award is valid for a maximum of 6 fall and spring semesters. Full or part-time status is for each semester acceptable but still counts toward one of your six semesters. The JAA achievement award does not apply to winter or summer sessions. You must maintain progress towards your JAA degree while at URI. Should you change your major, you will lose your achievement award. It is your responsibility to ensure your term bill is correct and it is your responsibility to notify transfer@etal.uri.edu (Transfer Resource Center) no later than the first day of classes if you believe an error has accidently occurred.
I understand that the loss of my award includes but is not limited to the language above. I understand there is no appeal process. It is your responsibility to work with the Transfer Resource Center (TRC) and academic advisor to ensure a timely graduation. Lastly, should there be a stoppage in continuing fall & spring semester education (leave of absence, military deployment, family leave etc), I must contact the Transfer Resource Center (TRC) no later than 15 days prior to the start of the semester for review and application of your previous award.
I understand withdrawals from the University and graduation from any undergraduate major will void the achievement award, but I recognize other scholarships may be available to me.

I understand that if I am eligible to receive a JAA academic achievement award of 10%, 20%, 30% based upon my final cumulative GPA at CCRI, that I must earn a minimum of a 3.0 each semester at URI.
